• Arrive on time for scheduled appointments

5. Appointments and Cancellations

Scheduling:

• All appointments are subject to availability
• Confirmation will be provided via phone, email, or the Zocdoc platform

Cancellation Policy:

• We request at least 24 hours advance notice for cancellations or rescheduling
• Repeated no-shows or late cancellations may result in a cancellation fee
• Patients who repeatedly miss appointments without notice may be dismissed from the practice
• We understand emergencies arise and will make reasonable accommodations when possible

Late Arrivals:

• If you arrive more than 15 minutes late, your appointment may need to be rescheduled

• Unpaid balances may be subject to collection proceedings

Treatment Estimates:
